— Our office policy is to collect payment in full for services at the time of the visit. This includes all co-pays, deductibles, and prior balances on your account. For insurance plans with a deductible, we collect a "deductible deposit." We will bill your insurance company for you, and after we receive the "Explanation of Benefits" from your insurance company, we will bill you for any residual balance. Deductible deposits will continue to be collected at each visit until your deductible is satisfied based on information received from your insurance company. — We make every effort to verify your Insurance eligibility prior to you arriving. Please bring your current insurance card to every visit. We participate with most insurance plans; however, the verification of benefits is not a guarantee of payment for rendered services. We recommend that you call your insurance company to verify participation as well as your benefits as the plan member. When you schedule appointments, we do require you to provide us with all insurance information at that time. If we are unable to verify coverage with your insurance company, you will be considered a private pay.

We participate with most PPO/HMO & HSA networks and if you do not have an insurance plan, we have cash pay discounts available to our families. Call our office for more information, even if you do not see your insurance company on this list. This list will not always be complete as changes with companies occur often.
