To best accommodate our patients, we schedule all of our office visits by appointment only and make every attempt to best accommodate your schedule. We are often able to schedule same-day appointments if you call us early in the day.
At Surprise Pediatrics, we believe that physician-patient relationship starts before birth. We encourage expectant parents to schedule a "Meet and Greet" session with our Practice Manager. It gives parents the opportunity to come in and meet our providers, tour the office, and ask questions. We recommend scheduling the appointment one to two months before your baby is due to arrive.
We also emphasize the importance of preventative practices and thus we emphasize regularly scheduled pediatric well checks in making sure that your baby or child is reaching his/her appropriate milestones and maintaining a healthy and happy life. It is best to always schedule out your well-child appointments at least two (2) weeks in advance to ensure you get the provider and time that you prefer.
If you are a new patient, we must have the parent or legal guardian bring the child to that first appointment. In our patient registration forms
, we ask you to grant permission to those you wish to be allowed to bring your child after that. At each appointment, we will also ask you to bring your insurance card, as well as a photo ID that includes your current address. You are also advised to bring the child's shot record to every well-child check. Please keep in mind we still ask whoever brings the child to the appointment to abide by our financial policy
and to show ID. In addition, we do not accept third party assignment nor do we recognize or enforce the terms of divorce decrees.
